Managing Wireframes and Screen Mockups

What workflows/tools have people used to manage wireframes and screen mocks for new projects? Problems that I've encountered in the past:
* The IA (wireframes) are different from what is present in the Photoshopped screen mocks and it's unclear which is the most current
* There are problems linking a wireframe/screen mock to a story. The files tend to be in a zip or PDF and you can only link to the overall document
* The wireframes tend to get uploaded to sharepoint and then never updated from there
What approaches have teams used to success when bridging the gap between design and development?

LucidChart is a tool for wireframing which is available on The Hub. You can import Visio diagrams and collaborate in real-time on wireframes, flow charts, mind maps, and org charts with your colleagues. You can publish diagrams as a web page and share a url which can be included in other documents. Changes to the diagram can be done in one place and the link is still valid. It's an extremely useful tool.
